My grandfather had the same name as my dad and my older brother had the same name as him. They went by their middle name which was Russell. I always referred to my grandfather as Da Da Russ and my dad just as dad. My brother we all called him Rusty.
It seems that when names are used in SR's, Jr's and 3rd's and so on nowadays they would refer to the Jr. and Junior and the 3rd as Tre/Trey.
I was wondering where they got to using the name John Boy? Was that something that was used in the deep South? Adding the name "Boy" to the end of the name if they share the same name with their father?

Well, living in WV, I would say that's a close counterpart to where the Waltons were set. I've never heard anyone use a name like that for a Jr. in the family. There was a boy I went to school with. He had the same name as his father, but rather than being a Jr. he had the Roman number for 2 after his name like this "II". And his nickname was Two.
In the movie Spencer's Mountain, it was Clay Boy instead of Clay Jr. I believe one of the earliest questions I asked on here was if Earl Hamner Jr. was called Earl Boy when he was growing up and the answer was no.

I am with you, and I have never heard the name Boy attached to a Junior. I grew up in Virginia and even be it that it was Northern Virginia most if they had the same name as their dad they went by Junior or in my brothers case by Rusty instead of Russell.

'John-Boy' was a better appellation than 'Sprocket' which, fortunately, appeared only twice in the series: 'The Hunt'.
Although Charlie Snead gave John-Boy the nickname 'Sprocket' when John-Boy was a child, it was never mentioned in 'The Homecoming: A Christmas Story'.
That makes me think that the nickname was made up specifically for the real pilot episode, 'The Hunt'.
