I have scanned and uploaded the letter I got from Carolyn, and if I did it correctly, you should be able to download, read it, and print it if you like. The form to send in is included in the letter You will find it at the top of the forum where you see "IMPORTANT INFORMATION" in red letters.

Yes, just send me a PM with your email address.
It is a PDF file, and when you click on it, it should open in Adobe Reader, if you have it on your computer.
